The Dkids Foundation (hereafter referred to as “Dkids”) is committed to meeting the highest standards of information privacy required by law, including the Personal Information Protection and Electronic Documents Act (PIPEDA) principles. The protection of your personal information is of great value to Dkids for persons living with diabetes (Hereafter “the Conference”).

 

When we use the words: “You”, “Yours” or “Applicant” in this Form, we mean the subject/owner of the personal information. This includes the information related to the child (hereafter “MINOR”) applying for the Conference or associated program activities as well as the personal information provided herein by the individual, parent or guardian.

 

We will collect information directly from you or we may collect it indirectly from other participants or from the Conference organizers or partners. Your personal information will be handled at all times in accordance with appropriate confidentiality standards and all applicable privacy laws.

 

 

PURPOSE AND SCOPE OF YOUR CONSENT FOR PERSONAL INFORMATION.

You consent to the collection, use and disclosure of your personal information, collected by The Dkids Foundation in relation to your purchase of tickets and participation in the Conference and/or associated program activities, and you agree with the terms and conditions below.

 

Your name will not be provided to third parties to be placed on any marketing mailing lists or sold to anyone for any purposes, unless otherwise noted.

 

Except as otherwise described above, Your personal information shall be disclosed to only those who have a need to know the information and the specific information disclosed shall be restricted to only that information relevant to the recipients’ need to know. Those who have a need to know include any The Dkids Foundation affiliate or third party providers acting on behalf of The Dkids Foundation or any of its affiliates.

 

 

PURPOSE AND SCOPE OF CONSENT FOR RECORDINGS.

You authorize Dkids to record your name, likeness, image, voice, sound effects, interview and performance on film, tape, or otherwise (all hereafter referred to as the "Recording"), edit such recording as Dkids may desire and incorporate such recording into the creation of promotional materials and videos, marketing and advertising materials, brochures, websites and social media and all and any other related materials (Hereafter the “Materials”). It is understood and agreed that Dkids shall retain final editorial, artistic, and technical control of the recording and its content. You agree the recording will be the property of Dkids or its assignees and that you have no interest in any intellectual property rights which may subsist in the recording.

 

Dkids may use, and authorize others to use, the Recording and any portions thereof in all markets, manner, formats and media, whether now known or hereafter developed, throughout the world, in perpetuity.

 

Dkids may share your recording with a third party who will act on behalf of Dkids to edit the recordings and to create, publish and distribute the Materials. Dkids has put in place the adequate contractual (and other) means to ensure that a comparable level of privacy protection is provided while the information is processed by the third party.

 

You hereby agree to indemnify and save harmless Dkids, its officers, employees, independent contractors, subcontractors, agents, and assigns from all cost, losses, damages, judgments, claims, demands, suits, actions, causes of action or other proceedings of any kind that they, or any of them, may incur for the infringement or alleged infringement of any intellectual property right or copyright based upon the use of anything provided by you is protected by any intellectual property or copyright protection.

 

Your personal information and recordings will be retained for as long as it is required for the purpose for which it was collected. After that period Dkids will make sure that your personal information is securely disposed of when it is no longer required.

TRANSFER OF PERSONAL INFORMATION TO COUNTRIES OTHER THAN CANADA.

Dkids may transfer personal information to jurisdictions whose governing laws respect the same or greater degree of information protection.

ACCESS TO INFORMATION AND WITHDRAW OF CONSENT.

We strive to keep our records of your personal information accurate. You may at any time designate any restrictions as to whom we may disclose your personal information or restrict the content of disclosure or ask for correction of any incorrectly stated information by sending written notice to the address below.

To withdraw your consent, you must provide written notice by registered post to Dkids using the contact details provided below. However your registration will not be able to be considered by Dkids and be considered void. Such notice(s) shall not affect any prior use by Dkids of your prior information.
